CloseKnit Wellness FAQ

Doing an annual wellness visit with TytoCare is clinically recommended and will allow your clinician to see real clinical grade measurements taken from home. This gives your clinician more tools to examine you, resulting in an accurate and relevant health assessment for you. 

As a valued CloseKnit member, you are eligible to upgrade your CloseKnit annual wellness experience to include a TytoCare kit at no additional cost to you!

Yes, your wellness visit will be with your usual CloseKnit clinician  

You can use the chat in the CloseKnit app to contact your clinician with any questions or concerns. 

You can schedule another visit from your CloseKnit app, or simply contact the Navigators in the CloseKnit app chat. If you got a TytoCare annual wellness kit, you will need to reschedule your visit within 72 hours, or return the kit. 

We recommend opening your kit as soon as you receive it, turning on the tablet and logging into the TytoCare app using the email and phone number you used to order the kit. This will allow your clinician to reach you when it’s time for your visit.

At the scheduled appointment time, your clinician will send you an invite using the TytoCare app on the tablet in order to start the visit.

Your care team has created a username for you. You can log into the app through SMS or email:

Enter your email and phone number and tap send.
You will receive an SMS with a code to log in.

To log in via email, tap Log in using a password.

If you didn’t receive these credentials, contact Tyto Support.

Enter your email and phone number and tap Send. When you receive an SMS message with a code, use this code to sign in. If you prefer to log in via email, tap Log in using a password. When you receive an email message with a password, use this password to sign in.

The TytoCare kit includes everything you need to do clinical grade exams from home during your annual wellness visit. It has a TytoCare device with a built-in thermometer and exam camera, as well as stethoscope, otoscope, and tongue depressor adaptors. It also includes a pulse oximeter, a pre-paired Omron blood pressure monitor and weight scale, and a tablet pre-configured with the TytoCare app. 

Your tablet has been configured to connect to the Internet using a cellular network, and you don’t need to connect to Wi-Fi. If this isn’t working, contact Tyto Support. 

Perhaps the tablet isn’t charged.

  1. Place the tablet on the tablet stand.
  2. Plug the power adapter into a nearby outlet and insert the power cord into the tablet.
  3. Locate the power button.
  4. Press and hold down the power button firmly for a few seconds until the screen lights up.
  5. Ensure the tablet is charged at least 5% before using it.

Your CloseKnit clinician will send you an invitation to join a visit directly from the TytoCare app. 15 minutes before your visit, simply log into the TytoCare app on the tablet and wait until your clinician sends you a notification to join the visit.   

15 minutes before the scheduled time for your annual wellness visit, unpack your kit on a table and place the scale on a hard surface (not a carpet).  

Log into the TytoCare app on the tablet by entering the email and phone number you used when you signed up. You’ll get a text message with a 6-digit code, use that to log into the app.  

Turn on the TytoCare device and wait until you see the green connected icon in the corner of the app screen. When it’s time for your scheduled visit, your CloseKnit clinician will send you a notification to the app to join the call. 

No, you will have a video visit done using the TytoCare app on the tablet you receive with your kit. 15 minutes before your visit, simply log into the TytoCare app on the tablet and wait for the clinician to initiate the visit.   

During your annual wellness visit, your CloseKnit clinician will guide you as to which exams you need to perform and explain to you how to do them.  

No, don’t worry about learning how to use the devices ahead of time. Your clinician will guide you through the exams during the visit. 

The only thing you need to do before your visit is to log into the TytoCare app on the tablet. If you would like to practice, you can do so at any time before your visit by just logging in.

After you complete the visit, turn off the tablet and carefully return all the items back into the box. Affix the included return label to the outside of the box, and tape the box shut.

If the device is unresponsive or stuck on a screen, press and hold the button on the top of the device for 10 seconds to force a shut down

When you’ve completed your annual wellness visit, carefully return all the devices and tablet into the box using the foam insert. Schedule a pick-up online at https://www.tytocare.com/providers/closeknit/schedulepickup.php or by calling 1-866-212-0123, or simply drop it off at your nearest UPS Store.
